Connect your safety forms to your AI assistant
Ask your AI assistant about this week's hazard reports, open incidents or missed pre-starts, and get answers straight from your FormSafely records. The connection is read-only, so the assistant can look but can't change anything.
What it does
FormSafely has an MCP server, a standard connector that AI assistants use to read data from other tools. Once it's connected, your assistant can:
Find your forms
List the forms in your organisation and open any one to see its fields, whether it's published, and how it's set up.
Look up submissions
Pull recent submissions for a form, filter by date, and open a single submission to read every answer.
Count and compare
Get submission counts by form and by day, so "how many near misses this month?" is a quick question rather than a spreadsheet job.
Export to CSV
Ask for a CSV of a form's submissions when you need the records in a spreadsheet or a report.
It's read-only. The assistant can't create, edit or delete forms or submissions, and it only sees your own organisation's data.
Who it's for
- Safety and operations managers who'd rather ask a question than build a report.
- Teams already using an AI assistant at work that want their inspection and incident records in the same conversation.
- Admins who prepare weekly summaries, toolbox talk topics or audit packs from form data.
Part of the Admin plan
AI assistant access comes with the Admin plan ($5 USD per admin user each month, with a 10-day free trial). Owners and admins of an organisation on the plan can create keys. People who only fill in forms don't need a key and stay free.
How to connect
1) Create a key
Sign in to the builder and open MCP / API keys from the sidebar or the Home page. Create a key and copy it. It's shown once, so keep it somewhere safe. You can have up to 10 active keys and revoke any of them at any time.
2) Add the server
In your AI assistant's connector or MCP settings, add a remote MCP server using the server address shown in the builder, and send your key as a bearer token in the Authorization header.
3) Ask a question
Start with something simple like "List my FormSafely forms". The assistant will pick the right tool and answer from your records.
Server address
Your MCP server address and key are shown in the builder under MCP / API keys (Admin plan), with a ready-to-copy example config.
Questions to try
- "Summarise this week's hazard reports and group them by location."
- "Which vehicle pre-starts were marked not safe to drive in the last 7 days?"
- "How many near miss reports did we get each day this month?"
- "Show me the latest incident report from the loading bay and list the actions taken."
- "Export this month's forklift pre-use inspections as a CSV."
- "Draft a toolbox talk topic based on the most common hazards reported this fortnight."
Keeping it safe
Your keys, your control
Each key belongs to one admin. We store only a scrambled fingerprint of it, never the key itself. Revoke a key in the builder and it stops working straight away.
Read-only and logged
Keys can only read data. Every request is logged and rate limited, and a key only works while your organisation is on the Admin plan.
